If an emergency has arisen for one of the following reasons then you maybe entitled to an emergency proxy.

• You have a medical emergency
• You are away from work
• Your photo ID is lost, stolen, destroyed or damaged.

• If you can't download or print a form, you can request one using our online form. - Note that it is now too late to post out a form in time for it to be returned before the deadline for the upcoming UK Parliamentary Election
